Instructions to Submit a Service Request

1. Select or Search for a Service Request Type: Report a wide range of local, non-emergency issues directly to City departments.

Note: Some City departments require a Fremont App account for Service Request types that may need follow-up for more information in order to respond to the request.

2. Where is the request?: Verify a Fremont location. You may be prompted to allow sharing your location with the browser to get an accurate location.

3. Tell us more details: Answer any additional questions related to the request or provide details in the description.

4. Attach photos or other media files: Optional.

5. Keep this request private: Optional if you would not like to have the service request be discoverable by other Fremont App users.

Once service request is submitted, follow-up will be sent via email if a Fremont App account was created or an email address was provided.

How do I access the Fremont App?

Fremont App is free to download on your smartphone or you can use it in your computer’s browser.
What is the Fremont App?

The City of Fremont launched Fremont App in May 2020. The Fremont App is a new CRM application that enables residents to report a wide range of local, non-emergency issues directly to City departments. By accessing Fremont App on a smartphone, tablet, or computer, residents can take a picture or video of local issues, such as potholes, tree hazards, unsafe and unsanitary malfunctioning streetlights or traffic signals, add a message to describe each respective issue, and send it directly to the proper City department.

The application uses geo-location technology to automatically route the problem’s exact location to the City, simplifying the reporting process for users. If users decide to create accounts they will receive notifications about the request’s status.

Am I required to submit a media file, such as photo, video, or audio file?

No, it is not a requirement to submit a photo, video, or audio file. It is highly recommended to give the Department assigned to respond has a better visual or perspective of the issue being reported. If no media files are added, please add as much details in the Description about the location and contact information for follow up.
Can I create a service request anonymously?

Yes, however, we encourage you sign up for a Fremont App account; you can submit a request anonymously and allow Fremont Staff to reach out if more information is needed in order to provide service to the location.
